Services to help older adults stay in their homes and maintain their independence.

Volunteers provide friendship and companionship by spending an hour a week with seniors who are at risk of becoming socially isolated. Participants and volunteers are matched by shared interests.

Provides in-home support to frail seniors with complex health needs to help them live at home longer, return home from hospital stays sooner or transition to long term care.

The program is offered jointly by professionals from the Home Care, Primary Care and Geriatric Programs who assist seniors in learning to better manage their health and make decisions that positively impact their quality of care and life.

Provides in-home services for people who require short or long term support to remain in their homes safely. Depending on individual needs, may include:
  • Nursing care: Health monitoring, IV therapy, injections, wound care, and patient teaching
  • Home support: Assistance with daily activities like bathing and dressing
  • Social work: Individual and family counselling for illness, loss or end-of-life issues
  • Dietitian services: Nutrition assessment and education
  • Physiotherapy: To maximize independence, function and mobility
  • Occupational therapy: Support for daily living, special devices, equipment or modification to home or workplace
  • Long term care: Assessment for nursing home admission
  • Palliative care: Comfort and support to live fully until the end of life
  • Adult day programs - enriching social activities for seniors
  • Adult protection - for vulnerable adults who need protection from neglect or abuse

Delivers library materials to individuals who are not able to visit a library in person. Items are chosen by library staff, and delivered to participants every six weeks.

Note: A similar delivery service is available for Community Care Facilities. Facility administration must contact PEI Public Library Service Headquarters to register for this program on behalf of residents.

Assists older adults and people with a terminal illness to find a new home for their pets. Also works with their families if they are unable to keep the pet. The program connects applicants to people who are looking to adopt a pet. Applicants are supported over the phone thorough out the process until a new home is found for their pets.

Provides frozen meal delivery to currently unserved or underserved communities that are not served by the Meals on Wheels PEI.

Provides assistance with navigating programs, supports and services for older adults, offered by:
  • Municipal, provincial, and federal government
  • Non-profit and community organizations
  • Private sector service providers
Can also provide assistance with completing forms.

Provides eligible older adults with financial assistance up to $1,800, depending on income, to help them pay for practical services that help them stay in their own homes. Services covered by funding include:

  • General home maintenance
  • Light house keeping
  • Snow removal
  • Grass cutting
  • Meal preparation
  • Transportation
  • Communication devices such as smartphones, computer modem, and WiFi devices
  • Complements supports provided to seniors by Home Care or family members for daily unmet needs
  • Lifeline Medical Alert System
