Eligibility:
Applicants must be experiencing one or more of the following:
-Need support to continue living at home
-Have been released from hospital and be recovering from surgery or an acute medical condition
-Have an ongoing health issue requiring more care than you or your family can provide

How to Access:
Complete and submit the Home Care Referral form to your local Home Care Office (form is available online) * Referrals can be made by the individual, family member, care provider or family doctor
