    Cornwall: Senior or have an identified need
    Evangeline: Senior or have an identified need
    Kensington: Senior or have an identified need, have a disability or housebound
    O'Leary: Senior or have an identified need, or have a disability
    Montague: Have an identified need or are housebound * Self-refer or can be referred by other health professionals
    Souris: Have an identified need or be housebound
    Summerside: Have an identified need
    Tyne Valley: Have an identified need such difficulty with or unable to prepare your own meals
