Navigation and Health Helplines

• Identify and work with health care providers and programs that provide support
• Help find and access community resources
• Help coordinate health care services required
• Answer questions about the health care system (both on- and off-Island services)

Offers free, confidential, non-medical information and referrals by phone and email to individuals with Parkinson's, their care partners, and healthcare professionals. Services include:

• Guidance
• Education
• Connections to resources
• Support for self-advocacy

• Connect with other health care professionals
• Locate programs and services that are available to help
• Assist with the discharge process from an out-of-province hospital

Provides one-on-one personalized help by a community service lead via phone. Includes:

• Listening attentively and with compassion to the challenges the caller is facing
• Navigating information specific to the caller's type of blood cancer or treatment
• Helping the caller get organized to manage and take charge of their care
• Identifying services that would be beneficial for the caller

Offers support for individuals with muscular dystrophy and their families through:

• Listening
• Emotional support
• Educational workshops
• Referrals to community resources
• Help with applications

• Building relationships of trust and provides guidance through the Health care system
• Coordinating care and assists with care transitions by serving as liaison between Mi'kmaq individuals, their treatment team, and the Health care system
• Educating about disease, treatment options, and follow-up care requirements
• Facilitating communication and cultural understanding with Mi'kmaq individuals and health care providers
• Advocating for improvements in PEI Health care system based on identified gaps facing Mi'kmaq individuals
• Supporting Mi'kmaq individuals by liaising with the Non-Insured Health Benefits program which provides eligible First Nations and Inuit clients with coverage for a range of health benefits that are not covered through other social programs, private insurance plans or provincial or territorial health insurance

Provides support and health care navigation services to Mi'kmaq individuals and families. Includes:

• Building relationships of trust and provides guidance through the Health care system
• Coordinating care and assists with care transitions by serving as liaison between Mi'kmaq individuals, their treatment team, and the Health care system
• Educating about disease, treatment options, and follow-up care requirements
• Facilitating communication and cultural understanding with Mi'kmaq individuals and health care providers

Provides six free online therapy sessions for adults with Intellectual Disability and/or Autism Spectrum Disorder experiencing mental health concerns and/or in need of behavior support.
