Provides navigation to individuals who have received a positive result from the fecal immunochemical test (FIT) and to those without a primary care provider who have applied for a colonoscopy through the online self-referral form, helping them access health care services, community support, and other resources.

Provides support to individuals who have been diagnosed with cancer and their families. The Cancer Patient Navigator can help to:

• Explain what to expect during appointments and treatments
• Find answers to questions about cancer screening and genetic testing
• Find answers to questions about care system
• Provide brief supportive counselling
• Connect patient with appropriate programs such as social programs, community groups, counselling, spiritual care, tobacco cessation support, the provincial breast prosthesis program and a dietitian
• Help prepare for and provide support during out-of-province travel for cancer related medical appointments and procedures
• Help coordinate care between provinces if moving to/from Epekwitk / PEI
• Connect to resources to help with financial needs and expenses for medications, travel or medical supplies

Note: Cancer patient navigators do not provide medical care.

Offers care kits that provides information and resources for individuals and their families who are newly diagnosed with type 1 diabetes (T1D).

There are two types of kits:

• Adult Care Kit

Trained volunteers provide personal support and information to individuals living with celiac disease or a gluten disorder.

Provides confidential telephone and email support to Individuals who are blind or partially sighted as well as their friends and family to help access the right programs and services such as support groups, and assistive technologies.
