Description:	Provides navigation to individuals who have received a positive result from the fecal immunochemical test (FIT) and to those without a primary care provider who have applied for a colonoscopy through the online self-referral form, helping them access health care services, community support, and other resources.
Eligibility:	-Individuals who have a positive FIT result will be contacted by the navigator -Individuals at increased risk for colorectal cancer without a primary care provider (due to family or personal history) can go directly to a colonoscopy for screening by applying through the online self-referral form. A navigator will reach out once the application is submitted
