Opportunities for individuals, families and/or couples to open their home to an adult with an intellectual disability to live together in a warm, home-like family atmosphere while enhancing community involvement and inclusion.

Compensation is established through the province's Financial Assistance Program and the AccessAbility Supports Program according to their eligibility and rates.

The organization monitors the individuals placed with Associate Families through follow-up visits, outreach and support.

Group homes and small option homes for individuals who may require personal care and supports with skills development and daily activities. Located across Queens County, small options homes have up to three residents and group homes have four to six residents. The homes offer:

  • Social and community involvement
  • A home for residents to call their own
  • Individualized plans that support each resident to reach their daily goals

Provides financial assistance to help cover the cost of major structural renovations to private homes. Eligible renovations include structural, heating, plumbing, and electrical systems, improvement for life safety, and accessibility upgrades for applicants with a permanent disability.

Maximum grants are based on combined annual income.

Fully equipped housing complex with seven units that provide supervised support for high functioning adults with intellectual disabilities.

Units are shared between two tenants and have two bedrooms, a bathroom, and kitchenette. There is a shared full kitchen, common dining area and living room. Tenants share chores such as meal planning, cooking and cleaning up.

Person-Centered Plans provide life skills and employment training and residents are supported to allow for an independent, secure lifestyle.

Provides three types of housing for individuals with intellectual disabilities:

  • Group Living: Located in the Montague area. Residents are encouraged to be as independent as possible but have the support of staff when required. Some are small options homes with 2-3 residents. Some facilities are staffed 24 hours a day, have day programming, and offer help with food and laundry.
  • Associate Family Program: People with intellectual disabilities can apply to live in a home with a local family who provides a nurturing, safe and supportive environment (pending availability).
  • Independent Living Centre: Provides support for 8 independent persons as well as a Small Options Unit ( a three-bedroom)

Housing options for clients with limited independent skills and a dependence on staff support to assist with their daily living needs. Locations across Summerside:

Winter Street: Group home with six permanent beds; one accessible bedroom on the first floor and five bedrooms on the second floor. Residents attend day services Monday-Friday.

Highland Avenue: Two bedroom apartment with 24-hour supervision.

Granville Street: Two apartment units with three bedrooms and 24-hour staff supervision. There is a common area for socialization and most residents eat their supper in the common room.

Barbara Weit Road: Rural group home with four permanent beds. One accessible bedroom on the first floor and three bedrooms on the second floor. Some residents attend day services Monday-Friday. 24-hour staff supervision.

Court Street: Two group home units with 24-hour staff supervision.

All locations are wheelchair accessible.

Housing option for clients with disabilities who are able to live fairly independently. Clients are responsible for the care of their apartment, possess independent living skills, and are responsible to purchase groceries for the preparation of their breakfast and lunch meals. Most clients receive their supper meal from Community Connections. Locations in Summerside:

Granville Street: Apartment building with six apartments connected with a common area, four of which have two bedrooms. Staff supervision in the common area through the weekday evenings and part of the weekend. Supper is served in the common area.

MacEwen Road: Apartment building with seven apartments connected with a common area. There are three apartments with one bedroom; three apartments with two bedrooms; and one apartment with three bedrooms that also has staff supervision Monday-Friday 4pm-8am and 24-hour supervision on weekends. For most residents, the supper meal is provided in the common area.

Court Street: Four individual affordable and accessible apartments on the upper level with a full kitchen/eating area, living room space, and an accessible bathroom. Laundry facilities are shared.
