Provides group and individual activities suited to seniors' personal abilities and interests while providing respite for family/friend caregivers. Some locations offer personal care services.

Provides in-home support to frail seniors with complex health needs to help them live at home longer, return home from hospital stays sooner or transition to long term care.

The program is offered jointly by professionals from the Home Care, Primary Care and Geriatric Programs who assist seniors in learning to better manage their health and make decisions that positively impact their quality of care and life.

Provides in-home services for people who require short or long term support to remain in their homes safely. Depending on individual needs, may include:
  • Nursing care: Health monitoring, IV therapy, injections, wound care, and patient teaching
  • Home support: Assistance with daily activities like bathing and dressing
  • Social work: Individual and family counselling for illness, loss or end-of-life issues
  • Dietitian services: Nutrition assessment and education
  • Physiotherapy: To maximize independence, function and mobility
  • Occupational therapy: Support for daily living, special devices, equipment or modification to home or workplace
  • Long term care: Assessment for nursing home admission
  • Palliative care: Comfort and support to live fully until the end of life
  • Adult day programs - enriching social activities for seniors
  • Adult protection - for vulnerable adults who need protection from neglect or abuse

Epekwitk / PEI has seven hospitals offering varying levels of care.

The two main referral hospitals providing inpatient, emergency, urgent psychiatric care, outpatient, community and specialty services are:

Individuals in need of urgent psychiatric care can go to the Emergency Room at Queen Elizabeth and Prince County Hospitals. They will be triaged and then see trained mental health staff.
Four community hospitals offer a variety of services that may include emergency, acute care, extended care, and community based services:

Provides individuals experiencing a mental health, addiction, or substance use crisis with streamlined, 24/7 access to a multi-disciplinary team of trained staff in an environment designed to meet emergency/urgent care needs.

Crisis conditions include:

  • A serious, immediate mental health, addictions or substance use concern
  • Risk of self-harm or harm to others
  • Psychosis
  • Emotional trauma, agitation, severe depression or anxiety suicidal thoughts

Performs specialized tests to diagnose, monitor, treat diseases and other health conditions of patients. Provides collection services and drop-off locations for blood, sweat tests, and bodily fluid.

Generally fluid samples such as saliva, blood and urine are collected at a doctor's office and forwarded to the lab for the result but sometimes individuals are sent to Laboratory Services to have collection done.

Provides specialized nursing and personal care services to people who can no longer live on their own, with family or with home care supports. Services are provided in both public and private long-term care nursing homes and include:
  • 24-hour nursing care
  • Room and board
  • Personal care
  • Medical services

Provides virtual health care to Islanders who are currently without a primary care provider. Patients can speak to a PEI physician virtually to have an assessment, diagnosis, e-prescription, lab requisitions, and more.

Health PEI is operating the program in partnership with a secure private healthcare platform, Maple.
Accessing the program does not remove patients from the waitlist or change their position on the waitlist.

Note: Book through Health PEI, not Maple directly. Private bookings ask for payment, but Health PEI-covered ones are free.

Provides professional medical assistance to people experiencing specific types of untreatable or irreversible medical conditions who wish to voluntarily and intentionally end their lives by administering a lethal dose of medication or prescribing medication for the patient to end their life through self-medication.

Note: There is a screening and assessment process to ensure awareness of other options, meeting of the criteria for medical assistance in dying and are capability of making this important decision.
