Provincial Geriatric Program Provided by: Health PEI

Provides specialized comprehensive assessments of older adults with the goal of improving their health and/or quality of life. The assessments focus on function to help support older adults to remain healthy and independent for as long as possible. Functional assessments may benefit older adults who are experiencing difficulties in one or more of the following area:

Memory
Depression
Mobility and gait
Daily care, e.g. bathing, grooming or cooking
Medication management

The assessment will be performed in a setting most appropriate to the client's needs. e.g. At home; in a long-term, community care or acute care facility; or at the Provincial Geriatric Program office

Palliative Care Program Provided by: Health PEI

Provides in-home and inpatient services to assist people experiencing a progressive life threatening illness and their families. Includes:

Medical, nursing and professional care
Assistance understanding your illness and treatment options
Helping you learn to express your wishes and personal goals and help you to achieve these goals
Pain and symptom management
Respite care for families/caregivers
Advance care planning
Ongoing bereavement support for families

Long-Term Care Provided by: Health PEI

Provides specialized nursing and personal care services to people who can no longer live on their own, with family or with home care supports. Services are provided in both public and private long-term care nursing homes and include:

24-hour nursing care
Room and board
Personal care
Medical services

Living with Changes in Thinking and Memory

Provides in-person support groups for individuals living with MCI or the early stages of dementia to connect with others living a similar journey and support each other. Meetings are held in Charlottetown. Features of meetings include:

Exchanging information and form friendships
Accessing the most current information about dementia
Learning and sharing practical tips for coping
Decreasing feelings of loneliness and isolation
Express feelings and receive support
Finding a sense of hope

Home Care - COACH Program

Provides in-home support to frail seniors with complex health needs to help them live at home longer, return home from hospital stays sooner or transition to long term care.

The program is offered jointly by professionals from the Home Care, Primary Care and Geriatric Programs who assist seniors in learning to better manage their health and make decisions that positively impact their quality of care and life.

Home Care - Adult Day Programs

Provides group and individual activities suited to seniors' personal abilities and interests while providing respite for family/friend caregivers. Some locations offer personal care services.

Note: Participants must arrange their own transportation to and from the program

First Link Learning Series

Provides education sessions to help people living with dementia, and their family members and friends to live as well as possible with the disease. Participants have opportunities to learn and share with others who are affected by dementia. The sessions include:

A comprehensive overview of Alzheimer's and other dementias
Skills needed to provide day to day care
How to understand changes in behaviour, personality, and communication
Coping strategies
Available resources and support
Care in the later stages

Provides education and support for individuals with living dementia and/or family caregivers from the time of diagnosis through ongoing treatment.
