Provides specialized comprehensive assessments of older adults with the goal of improving their health and/or quality of life. The assessments focus on function to help support older adults to remain healthy and independent for as long as possible. Functional assessments may benefit older adults who are experiencing difficulties in one or more of the following area:
Memory
Depression
Mobility and gait
Daily care, e.g. bathing, grooming or cooking
Medication management
The assessment will be performed in a setting most appropriate to the client's needs. e.g. At home; in a long-term, community care or acute care facility; or at the Provincial Geriatric Program office

Referral by family doctor required * Call the Provincial Geriatric Program office for information about how to proceed if you do not have a family doctor
