Eligibility:
Adults with a primary diagnosis of mental illness and are age 18-65 years at the time of application * Can be struggling with or in recovery from mental illness
Fees:
No fees
How to Access:
Call, email or drop in for a referral form to complete * Can also book a tour of the program prior to or after completion of the referral form
Documents Required:
Referral form completed by an eligible health care provider and Release of Confidentiality Form completed by the applicant. Examples of eligible health care providers include: Psychiatrist * Doctor * Nurse * Mental Health Counselor * Social Worker
