Patient Navigator Service

Provides patients and their families and caregivers with guidance to help find and access services needed in PEI's health care system. Patient Navigators can:

Identify and work with health care providers and programs that provide support
Help find and access community resources
Help coordinate health care services required
Answer questions about the health care system (both on- and off-Island services)
