Provides specialized services to seniors who are experiencing complex mental health problems and support to their families and/or caregivers. Services are offered in-office as well as through community outreach to clients living at home or in community care. Teams work collaboratively with other health care providers like Home Care, physicians, the Provincial Geriatric Program, and Inpatient Psychiatry Units to provide the following services:

Description:	Provides specialized services to seniors who are experiencing complex mental health problems and support to their families and/or caregivers. Services are offered in-office as well as through community outreach to clients living at home or in community care. Teams work collaboratively with other health care providers like Home Care, physicians, the Provincial Geriatric Program, and Inpatient Psychiatry Units to provide the following services: Assessment Diagnosis Treatment Condition management and support
Eligibility:	Older adults who are experiencing complex emotional, functional or behavioural problems as a result of: -New onset or existing serious mental illness complicated by the aging process (e.g. mood disorders, psychosis, anxiety OR -Dementia with psychiatric complications
Fees:	No fees
How to Access:	Self-referrals or referrals by family/friends/community members can be made by contacting the local Community Mental Health office * Health care providers can refer by submitting an Adult Services Referral Form (available on website) to Community Mental Health
Documents Required:	PEI Health Card * For health care provider referrals, completed form must be submitted
