Description:	Promotes housing stability for individuals/families by developing case plans, teaching self-sufficiency and offering life skills development. Connects participants to the community through available resources. Services include: Skill Building Goal Setting Housing Search Advocacy Referrals Case Management Assessment and Treatment Planning
Eligibility:	Open to eligible Epekwitk / PEI residents 18 years and older who are experiencing homelessness, have inadequate housing or are facing eviction
Fees:	No fees
How to Access:	Referrals accepted from self-referral, current community resource such as Provincial Housing Navigator, Probation Officer, Social Worker, Social Programs Coordinator, Physician or Provincial Correctional Centre
