Provides support to individuals who have been diagnosed with cancer and their families. The Cancer Patient Navigator can help to:
- Explain what to expect during appointments and treatments
- Find answers to questions about cancer screening and genetic testing
- Find answers to questions about care system
- Provide brief supportive counselling
- Connect patient with appropriate programs such as social programs, community groups, counselling, spiritual care, tobacco cessation support, the provincial breast prosthesis program and a dietitian
- Help prepare for and provide support during out-of-province travel for cancer related medical appointments and procedures
- Help coordinate care between provinces if moving to/from Epekwitk / PEI
- Connect to resources to help with financial needs and expenses for medications, travel or medical supplies
