Description:	Assists veterans and their families who are homeless or at risk of impending homelessness by providing a case worker who can support them in attaining life stabilization.
Eligibility:	PEI residents age 18 years and older who have an active Veteran Service Number including RCMP veterans
Fees:	No fees
How to Access:	Referrals can be made using the online application on the John Howard Society, PEI website, self referral or referral via another resource connected to the individual seeking service
Documents Required:	Proof of an active Veteran Service Number
