Description:	Provides low-income individuals living with cancer an annual travel allowance to help cover the costs of travelling to cancer related appointments. One travel allowance is granted per year per person. Note: Recipients of the Social Assistance Program or Accessibility Supports (Disability Support Program) in Epekwitk / PEI are not eligible for the Travel Treatment Fund.
Eligibility:	Eligible applicants must: -Be undergoing active cancer treatment - Have at least 10 appointments per year related to treatment OR live more than 200 km away from the treatment centre -Have household combined gross annual income is less than $44,500 Note: Recipients of the Social Assistance Program or Accessibility Supports (Disability Support Program) in Epekwitk / PEI are not eligible for the Travel Treatment Fund.
Fees:	No fees
How to Access:	Have the application form completed by a health professional and return it by mail, fax, or email
Documents Required:	Most recent Notice of Assessment from Canada Revenue Agency (CRA) for all members of the family unit over 18 years of age * If applicant or their partner's financial situation has changed during the year, proof of current family unit income is required * Signed Direct Deposit Enrollment Form with a void cheque (if applicable)
