Description:	Provides speech language pathology services for adults who are experiencing: Voice disorders Neurological conditions (for example: Parkinson's, ALS) Stuttering Cognitive communication (for example: brain injury, dementia) Augmentative or alternative communication Gender-affirming voice and communication services
Eligibility:	Ages 18 years and older who are experiencing issues with speech language
Fees:	No fees
How to Access:	Call, mail, drop off or email referral form or apply online
Documents Required:	Completed referral form * A Referral from a health care provider or a self-referral
