    Provides education, assessments, and preventative nail and foot care for people living with diabetes with a moderate or higher risk of foot problems. Includes:

    • Foot and nail assessments by a foot care nurse
    • Nail care, callus care, and corn care
    • Education and promotion of self-care or caregiver-provided care
    • Education to prevent common foot issues
    • Foot care nurses provide surface care only and can recommend medical follow-up for infections, wounds, or other issues

    Note: Home visits are not offered.

    Service Description

    Provides education, assessments, and preventative nail and foot care for people living with diabetes with a moderate or higher risk of foot problems. Includes:

    Products specifications
    Eligibility:Eligible patients must:

    -Be diagnosed with diabetes
    -Have a moderate or higher risk for diabetes-related foot problems, as determined by screening from a health care provider
    -Be able to travel to a clinic
    -Be able to get into the foot care chair independently
    -Have insufficient insurance coverage for foot care (Insurance covers less than 80% of foot care needs)
    Fees:No fees
    How to Access:For those with a family doctor or nurse practitioner, book an appointment with them for a risk screening * For those without a primary care provider, contact a local primary care office for a risk screening
