Offers education, support and diabetes management advice for individuals who are living with or at risk of developing diabetes. Services offered include:
Getting Started classes
At Risk classes
Individual assessment and counselling
Insulin instruction
Insulin pump assessment, education and ongoing support
Eligibility:
Individuals diagnosed with diabetes * Individuals at risk for type 2 diabetes or who have been diagnosed with pre-diabetes

No fees
How to Access:
Call your nearest office to self-refer * Family physicians and nurse practitioners can also make referrals to the program
