Description:	Offers information, education and support on: COPD prevention and smoking cessation Early detection and diagnosis Intervention and COPD management Follow-up and monitoring
Eligibility:	Open to individuals who are at risk for or have been diagnosed with COPD and their families
Fees:	No fees
How to Access:	Call your nearest COPD clinic to self-refer for education and support services * Health care professionals can also refer their patients * Only a doctor or nurse practitioner can make a referral for spirometry (a breathing test)
