Individuals residing in PEI can apply to have their name legally changed. When the process is complete, the individual will receive a Change of Name Certificate. The new name will appear on any birth certificates issued in PEI after the change of name process is complete.
Changing a surname for marriage does not require a legal change of name unless wanting to change the name on a birth certificate. A marriage certificate can be used to change identification documents.

Eligibility:
Applications for individuals 18 years and over: Must have resided in PEI for at least three months * Are not in the lawful custody of another person
Applications for individuals under 18 years: Legal guardians can submit an application. They must: Have resided in PEI for at least three months * Not be in the lawful custody of another person

Fill out required forms and submit an application * Forms and application available online
Documents Required:
Completed application form(s) * Written consent of child and legal guardians of any child aged 12-18 years, whose name is to be changed * Detailed birth certificate including parent information if the person whose name is to be changed was born outside PEI * Proof of PEI residency (PEI Health Card, citizenship card, etc.) * All previously issued birth certificates, marriage certificates and change of name certificates from another province, territory, state, or country
