    New to PEI
    Individuals residing in PEI can apply to have their name legally changed. When the process is complete, the individual will receive a Change of Name Certificate. The new name will appear on any birth certificates issued in PEI after the change of name process is complete.

    Changing a surname for marriage does not require a legal change of name unless wanting to change the name on a birth certificate. A marriage certificate can be used to change identification documents.

    Service Description

    Documents Required:Completed application form(s) * Written consent of child and legal guardians of any child aged 12-18 years, whose name is to be changed * Detailed birth certificate including parent information if the person whose name is to be changed was born outside PEI * Proof of PEI residency (PEI Health Card, citizenship card, etc.) * All previously issued birth certificates, marriage certificates and change of name certificates from another province, territory, state, or country
