Eligibility:	Residents of Canada with a confirmed diagnosis of MS who meet the low income cut-off (LICO) * Assessment of need by a healthcare professional
Fees:	No fees
How to Access:	Submit the Quality of Life Application Form online * Have a healthcare professional submit the Quality of Life Health Care Professional Assessment Form online * Does not provide 'after the fact' funding, applications must be submitted prior to purchase
