Description:
A funding support program that provides financial assistance to Islanders who are accessing in-vitro fertilization (IVF), intrauterine insemination (IUI) and associated prescribed medications at out-of-province Canadian fertility clinics.
Note: Approved applicants must submit expenses that occur after their approval date. Retroactive expenses will not be eligible for reimbursement.
Eligibility:
Applicants must:
-Be permanent PEI residents as defined by the Health and Dental Services Cost Assistance Act
-Have a valid PEI Health Card
-Have filed their most recent income tax return in PEI
Fees:
No fees
How to Access:
Mail completed application form (available online) for review to determine eligibility * If eligible, the applicant will receive an approval letter identifying their maximum eligible reimbursement funding
Documents Required:
Completed application form * PEI Health Card * Social Insurance Card * Private health insurance information (if applicable) * Spouse's Social Insurance Number and private health insurance information (if applicable) * Letter from fertility clinic confirming candidacy for IVF or IUI (if not currently a client of a clinic this can be submitted later) * Submit all eligible receipts to private insurance first if applicable; remaining expenses can then be submitted to the Fertility Support Program for reimbursement
