Eligibility:	PEI residents of all ages living with type 1 diabetes who: Reside in Epekwitk / PEI -Have a valid PEI health card Have filed most recent income tax return in Epekwitk / PEI -Meet certain medical criteria

How to Access:	Complete self-assessment form Individuals with private insurance need to contact their provider to determine what percentage it covers for insulin pumps and supplies * Use Insulin Pump Payment Calculator to determine break downs of payment details * Contact health care provider to complete an intake assessment See website for full details
