Eligibility:	PEI residents who are receiving kidney dialysis or diagnosed with chronic renal failure
Fees:	No fees
How to Access:	Referral from nephrologist or internist * Referral form can be filled out online * Physicians can also make a referral provided they include a consultation report from a nephrologist or internist
Documents Required:	Valid PEI Health Card * Completed form
Language:	English
