Eligibility:	PEI residents and their family who met the financial threshold (see website for details) or, who are receiving financial assistance from the Department of Social Development and Housing
Fees:	Sliding scale dependent on household income
How to Access:	To apply, mail the completed application form to Dental Public Health PO Box 2000 152 St. Peter's Road Charlottetown, PE C1A 7N8 * Application form is available online, by phone or in person at Four Neighborhood Centre and Summerside Dental Public Health Clinic
