Provides a community-based home care service designed to support older adults who are Francophone or bilingual, as well as their caregivers. Includes:

  • Physical exercises
  • Cognitive simulation to arts, crafts, and social interaction
  • Respite for caregivers
Products specifications
Eligibility:Francophone and bilingual adults and their caregivers who live in the Evangeline area who
-Are ages 65 years and older
-Can participate in group settings and transfer with minimal assistance
-Are at risk of losing independence but can safely live at home
-Have chronic or complex medical conditions
-Show signs of cognitive decline or experience social isolation
-Have caregivers in need of respite
Note: Those in long-term care or awaiting placement in hospital are not eligible.

How to Access:Referrals can be mailed, dropped off at either the Summerside or O'Leary offices, or faxed to 902-888-8439 * The referral forms in English and French available online
Documents Required:Completed referral form by individuals, family members, health care providers, or family doctors
