Spinal Cord Injury - Seniors Navigator

Empowers older adults with mobility issues to live independently. Navigators assist in setting goals, reviewing supports, and developing plans with the individual and their support network. Support is available online and in-person, at home, or in any preferred space. Supports include:

• Identify needs and possible solutions
• Set and reach goals
• Learn new skills
• Help find affordable housing and supports
• Help connect with peer support and other available social programs
• Find accessible transportation options
• Work with service providers such as nurses and home support workers
• Help navigate government financial supports and more
