Description:	Provides youth in the care of the Director of Child Protection and youth who have aged out of care of the Director of Child Protection with a variety of supports to assist with the successful transition to adulthood. Eligibility:	Youth ages 14-17 who are in the care of the Directory of Child Protection in Epekwitk / PEI * Young adults ages 18-21 who were in the care of the Director of Child Protection in Epekwitk / PEI
Fees:	No fees
How to Access:	Self-refer or obtain a referral from a social worker
Documents Required:	Complete referral form
